How to refer

Patients can be referred to the Bath Oral Surgery Clinic using the online form here or by downloading and printing a referral form. All referrals will be dealt with quickly.

We do ask that any relevant radiographs are posted following the online referral process or included with the paper form. All information provided to the clinic is treated in the strictest confidence.

In accordance with national guidelines set out by the Department of Health and the Royal College of Anaesthetists, a short consultation appointment is necessary for patients referred for surgical treatment. During this appointment, medical assessment is carried out, the procedure is discussed with the patient and information sheets provided. This enables valid, informed consent to be obtained prior to commencement of treatment.

The fees page can be used as a guide to likely costs for patients.

Following treatment, patients are supplied with complete postoperative instructions and medications as appropriate, together with contact numbers should there be any concerns.

All patients are discharged back to the referring practice.

Communication is important to us. Correspondence confirming the appointment date and time will be sent to both patient and referring practitioner and after surgery, a following letter outlining the treatment carried out. All original radiographs will be returned to the referring practice once treatment has been completed.
